KENTUCKY RIVER AREA DEVELOPMENT DISTRICTPOSITION DESCRIPTIONHOMECARE/MEDICAID CASE MANAGER The Kentucky River Area Development District, an eight county planning and development agency, is accepting applications for a Homecare/Medicaid Case Manager. This position, under the supervision of the Human Service Director, provides case management in homes with clients as per program guide lines, perform other duties as assigned by supervisor. A competency test will be required before Interviewing with the Personnel Committee of the KRADD. Examples of Duties include but are not limited to: Conducts all activities required to support assessment and case management for Homecare/Medicaid Clients living in their assigned counties to provide services to individuals who are at risk of being institutionalized due to physical or mental problems and do not have enough formal and/or informal support systems to meet their needs. MINIMUM QUALIFICATIONS: A Bachelors degree in social work or closely related field with a minimum of a two (2) years professional experience in working with the elderly or an MSW with six (6) month’s experience. A licensed nurse who is either a licensed practical nurse or a registered nurse with two (2) years professional experience in direct care to the elderly while an employee of a home health agency, long term care facility, public health agency, or social services agency. Applicant must have good working knowledge of computers and software applications and posses a valid drivers licenses and dependable vehicle. Possess good written and oral communication skills; knowledge of reporting and vital record keeping requirements; ability to establish and maintain effective working relationships with other employees and clients who could have mental or criminal backgrounds. Criminal Records check required. Salary starting at $12.00 per hour plus excellent benefits.
