HAZARD — Primary Care Centers of Eastern Kentucky has announced that their Diabetes Center of Excellence is now utilizing Welkin’s advanced technology to elevate the care that is given to patients with Type I, Type II and gestational diabetes. This platform allows treatment professionals and patients the ability to communicate with each other at any time, from anywhere, using their preferred method of communication, be it texting, smartphone applications, email or telephone call.

Primary Care Centers of Eastern Kentucky established a partnership with Welkin Health three years ago to help facilitate the Primary Care Centers’ diabetes program. This new, modernized platform will extend the collaboration and support all care coordination needs for the center.

PCCEK was founded in 2003, offering quality healthcare to residents of Eastern Kentucky. The Diabetes Center of Excellence currently treats 750 diabetes patients ranging in age from 11 years old to over 80.
