David Tyson Fugate, 37, of Bulan, died Tuesday, June 5 in Manchester, Ky.
He is preceded in death by his father, David Lee Fugate.
He is survived by his mother, Geneva Fugate; daughter Chelsea Lynn Wooton; son Brandon Wooton; sister Shontrelle Fugate; girlfriend Mary Deaton; and a host of family and friends.
Arrangements were handled by Maggard Mountain View Chapel of Hazard, where services were held on Saturday, June 9. Interment was at Combs Cemetery on Strongs Branch.
